Health & Wellbeing Survey
Drayton Medical Practice along with most Practices in Shropshire have been asked to partner with Shropshire Council in promoting their Health & Well Being Survey.
This is part of Public Health’s Place-based Joint Strategic Needs Assessment. Which focuses on a particular geographic area to identify, understand, and address the key concerns and issues specific to the communities living there.
To help promote the survey and garner as good a response as possible, we have sent text messages to our patients with the link to the survey (as below). We encourage you to complete it before the closing date of December 16th.
