Child Immunisation

It is important that your child is fully immunised, so we have provided a schedule so that you can check that your child is kept up to date.

For comprehensive information on childhood immunisations in the UK, including translations of the schedule and information on all the vaccines and diseases protected against, please visit the NHS immunisation website.

If you are unable to attend an immunisation clinic, please remember to call us so an alternative date can be arranged.

Each vaccination is given as a single injection into the muscle of the thigh or upper arm.

Falls Prevention Service

Who We Are

An enthusiastic team of Falls Prevention Practitioners and exercise specialists, comprising of physiotherapist, occupational therapist, nurse, rehab techs and exercise instructor.

 What We Do

If you have fallen, are at risk, or are fearful of falling, and live within the boundary of the Shropshire County PCT, we will offer you an assessment. The assessment is to try and find out what is making you more likely to fall.

With your consent we then devise an action plan to reduce your risk of further falls. This might include:

  • Asking your G.P. or pharmacist to review your medication
  • Looking at ways to make your home environment safer
  • Inviting you to join a Falls Prevention Programme
What Is A Falls Prevention Programme?
  • This is an exercise programme specifically designed to improve your strength, balance and confidence.
  • Each programme is of 12 weeks duration.
  • Each week there is an opportunity to discuss a topic that is related to reducing your risk of falls.
  • You may be offered a Home Exercise programme if this is more appropriate.

Exercise programmes are held throughout the year in:

Bishops Castle
Bridgnorth
Church Stretton
Ludlow
Market Drayton
Oswestry
Shrewsbury
Whitchurch

What Other Services May Be Involved In Your Care?

You may be seen in DAART, or in a consultant clinic. Here you will have a medical falls risk assessment to rule out any medical conditions that may be contributing towards your risk of falls. The doctor may also assess your risk of having fragile bones.

You may be seen by the integrated care teams – these include district nurses, community physiotherapists, and occupational therapists who are also able to carry out a falls risk assessment.

Some members of the teams are also trained to do a home exercise programme to improve your strength and balance.

 Referrals

We maintain close links with services already mentioned and the Shropshire hospitals, GP’s, social, emergency and community services.

  • Anyone can refer into the Falls Prevention Service – with your consent.
  • You may also refer yourself.

Falls are not an inevitable part of growing older. Help us to help you remain healthy, active and independent.

It is committed to provide a high quality service by qualified and supervised staff that are themselves committed to professional development.

 What Is Counselling?

Counselling offers you the opportunity to talk to someone in confidence about your problems or issues that concern you. It provides you with the time and space to think and reflect with someone outside your circle of family and friends. It is a therapeutic approach that seeks to help you explore and understand current difficulties, and to make choices in your life.

Counselling aims to help individuals take responsibility for their own decisions, become more aware of their personal resources, more skilled at coping with difficulties and more self reliant in the future.

 What Do We Do?

The counsellor will listen to you carefully as you talk about your thoughts, feelings, other people, the situation and respond to you in a non-judgmental, non-critical way. They will respect your values, choices, and lifestyle. They will help you to find ways to understand what is happening and tackle the problem.

 Who Are The Counsellors?

There is one part-time counsellor who works for the Department of Psychological Therapies, who is qualified and experienced in different types of therapy, and two trainee counsellors.

Health Visiting Service Top

Your health visitor is a qualified nurse who has had extra training to become a health visitor. Part of their role is to help families, especially families with babies and young children, to avoid illness and keep healthy. Health visitors are part of a team offering screening and developmental checks as part of the Healthy Child Programme.

Your health visitor will usually contact you for the first time around 10 to 14 days after your baby is born. They will then be available for your family to discuss any concerns you might have, including breastfeeding or general baby and toddler feeding, and your child’s behaviour, up to five years of age. They will be able to offer advice and suggest where to find help. They may also be able to put you in touch with groups where you can meet other parents.

Your health visitor is available to you as well as your children, and can help if you are feeling anxious or depressed.

Your health visitor can visit you at home, or you can see them by appointment at the Child Health Clinic, or at Surestart Children’s Centres.

Child Health Clinics Top

Child health clinics are run by health visitors who offer health and development reviews. You can talk about any problems to do with your child, but if your child is ill and is likely to need treatment, you should go to your GP.
