Please let us know if you cannot attend

You can only cancel appointments that you hold with the Medical Practice, using the methods below. It is important to cancel an appointment if you are unable to attend. The appointment can then be used by someone else who needs it.

• You can call our Cancellation line on 01630 692900. This is a voicemail service. Please leave your name, date of birth and appointment details.
• You can cancel your appointment at any time via your online account.
• During normal opening hours, you can call 01630 652158 and choose option 2 for the cancellation line.
• You can complete the online appointment cancellation form, below.

Where possible, reminders for appointments are sent via text (SMS) to patients with a mobile phone number recorded. Please let us know your up-to-date number using this form. You can also use this form to opt out of receiving text messages from us. You can also complete a form when attending the Practice. Please ask at reception.

Any appointments with the Community Services e.g., District Nurses, Health Visitors or Physio must be cancelled directly with that department. You can contact Community Reception on 01630 653881.
