North East Shropshire Community Team

The team provides therapeutic intervention for complex medical conditions, rehabilitation following discharge, urgent treatment to avoid hospital admission and palliative care in the community.

Therapeutic interventions provided by:

  • Physiotherapist
  • Occupational Therapist
  • Speech and Language Therapist
  • Rehabilitation Technician
  • Age UK Co-Ordinator

The service operates Monday – Friday 9am to 5pm (excluding Bank Holidays)

Please note most staff work part time.

Please note our service is restricted for housebound service users only. Service users who do not meet the criteria can be referred to Outpatient Physiotherapy or the Social Services or the Independent Living Partnership for an Occupational Therapy assessment.

   Physiotherapy:

  • Mobility assessment and provision of mobility aids
  • Therapeutic exercise programmes
  • Falls prevention
  • Chest Therapy
  • Complex moving and Handling Advice

   Occupational Therapy:

  • Assessment for equipment provision for the home (specifically for beds, chairs and toilets)
  • Complex moving and handling assessments
  • Assessment for stair and grab rails
  • Trusted Assessor equipment provision
  • Seating Assessments

   Speech and Language Therapist Assessment:

  • Swallowing Assessment and Advice
  • Speech difficulties
  • Assessment and advice communication aids

   Age UK Co-Ordinator:

  • Advice on Age UK services and support
  • Assessment for low level equipment provision
  • Support with charitable application when appropriate
  • Wheelchair Assessments
  • Advice with general community based services and support
  • Onward referral for benefit checks
